● Menopausal symptoms

A review of the Cochrane Database of Systematic Reviews (containing 16 studies with 2027 participants) states that as of 2012, there is insufficient evidence to conclude that black cohosh helps improve menopausal symptoms (due to high heterogeneity, insufficient number of studies, and lack of statistical significance).

However, another double-blind controlled study (8 weeks, 84 postmenopausal women) published in 2013 showed that compared to placebo, black cohosh helped reduce menopausal symptom scale scores and showed variable improvements in other subscales such as vasodilation, mental, physical, and sexual function symptoms.

 

● Sleep disturbances (menopause-induced)

A double-blind controlled study (6 months, 48 postmenopausal women) found that Black cohosh extract powder helped improve sleep efficiency, reduce wake after sleep onset (by 15.8%), and enhance the sleep quality index (though more empirical confirmation is needed due to the small sample size).

 

● Bone health

A double-blind controlled study (12 weeks, 62 postmenopausal women) showed that black cohosh was beneficial to bone metabolism, stimulating osteoblast activity and aiding bone remodeling.

However, another controlled study found that the addition of black cohosh did not enhance the effects of exercise training alone regarding benefits to bone density and lean body mass, indicating a need for further confirmation.
