Introduction:
Dextran powder is a macromolecular glucose polymer produced by the fermentation of sucrose. Due to the different number of polymerized glucose molecules, different molecular weight products are produced. There are four types of dextran as following.
● High molecular weight (average molecular weight 100,000 to 200,000)
● Medium molecular weight (60,000 to 80,000)
● Low molecular weight (20,000 to 40,000)
● Small molecular weight (10,000 to 20,000).

The most frequently employed dextran powder varieties are:Dextrose 20, Dextrose 40, Dextrose 70.
1. Molecular weight size:
● Glucose polymer 20 has a molecular weight between 10,000-20,000
● The molecular weight of dextran 40 is between 20,000-40,000
● Dextrose 70 has a molecular weight between 60,000 and 80,000 molecules.
2. Rate of elimination from the body:
● Dextrose 20 has a shorter half-life in the body, lasting around 3 hours, and is eliminated more easily in urine.
● Dextrose 40 has a slightly longer half-life of roughly 24 hours, with the kidneys excreting about half of it.
● Dextrose 70 has a relatively longer duration of action, about 12 hours, and about 60% of the drug is excreted in about 24 hours.
3. Clinical uses:
● Dextrose 20 is mainly used to improve microcirculation, prevent or eliminate intravascular red blood cell aggregation and thrombosis, etc. It is also used to expand blood volume transiently.
● Although dextrose 40 can also expand blood volume, but its action time is shorter, and has the function of osmotic diuresis and improve microcirculation, so it is often used in the treatment of shock, cerebral thrombosis and myocardial infarction patients.
● Glucose polymer 70 is mainly used as a plasma substitute to prevent or relieve hemorrhagic shock, traumatic shock and burn shock.

Applications:
1. Pharmaceutical industry:
It is similar to plasma proteins and globulins. Colloidal properties are similar to plasma, with no adverse effects on cell structure and function. It has nutritional effect by hydrolyzing into glucose in the body. Therefore, dextran has the pharmacological effects of expanding blood volume, improving microcirculation, diuresis and swelling, etc.
